Cette édition fut imprimée deux fois par l'imprimerie Vallée : en 1865, à l'adresse du 15, rue Bréda; la seconde, en 1870, avec faux-titre et titre renouvelés à l'adresse du 16, rue du Croissant.
CONTRECOLLÉE SUR UNE PAGE EN VIS-À-VIS DU TITRE: PORTRAIT D'HECTOR BERLIOZ PHOTOGRAPHIE ORIGINALE, tirage albuminé, contretype, par François-Marie-Louis-Alexandre GOBINET DE VILLECHOLLE (dit Franck, 1816-1906).
